HMA’s Care Navigator program is designed to engage with members at various points in their healthcare journey to help them make educated decisions about their care and empower them to achieve well-being. The goal of the Care Navigator is to assist the member in building the skills needed to make good decisions and influencing influence behavior change that leads to lower costs and better outcomes. We are looking for a talented professional to join our Care Navigator team. The Care Navigators work closely with our Care Management and Customer Care teams to coordinate member patient care, remove barriers for members, increase member engagement, lower members’ stress and fear related to using their benefits, and drive better member experiences.
Use judgement and discretion to Prioritize work items from various sources, identify members who would benefit from Care Navigator support from a variety of sources including authorization requests,; analytics reports, and referrals from both internal and external partnersCare Management referrals, Client and /or Broker referrals; Client Success referrals; patient or family member referrals for Care Navigator outreach Complete outreach calls to patients Proactively outreach to members who would benefit from Care Navigator support; prioritize members with consideration to needs, timing (of member healthcare plans), and potential value to member and plan Promote Care Navigator services and build rapport with members; utilize the skills of a Trusted Advisor and expertise in healthcare administration to identify areas of concern and opportunity for members; anticipate member needs and identify resources available that would benefit and support the member, and with consideration given to plan priorities and requests. Identify areas of concern / anticipated needs (transportation, support care, social issues, etc.) and identify resources, build rapport with providers to ensure timely communication regarding member needs Utilize reports and dashboards to identify and engage with members with steerage opportunities Identify and promote steerage opportunity for members to deliver high-quality, high-value care Create and maintain strong collaborative relationships with vendor partners for products offered by plans Engage in personalized consultations with members Educate patients regarding claims, benefits, and options available under the plan and in their community Perform follow up as needed Review claims for payment integrity and patterns of overutilization Provide data to compile reports
High School diploma required 1-3 years of experience in an administrative healthcare role Proficiency in medical and dental terminology preferred Excellent verbal and written communication skills required Excellent customer service skills required Excellent time-management skills needed Detail oriented and strong organization skills needed Ability to work well both independently and as part of a team Ability to identify and resolve complex issues and problems Ability to perform well and multi-task in a busy, high pressure environment Moderate computer skills (email, MS Word, MS Teams, etc.)
The base salary range for this position in the greater Seattle area is $28.85/hr - $35.57/hr for a level I and varies dependent on geography, skills, experience, education, and other job or market-related factors.
